Thank you for using Sojorne. Sojorne is a platform that supports caregivers of children or adults with special needs, disabilities or chronic health conditions to live better lives. We partner with families to address their care, coordination and community needs. Sojorne provides services through its mobile application, web application and through webinars and small groups hosted through its website.
